1 CHILD PARTICIPATION PERMISSION, WAIVER, RELEASE AND INDEMNITY AGREEMENT
(“RELEASE”)
1.1 I/We, am/are the parent(s) or legal guardian(s) of the abovementioned Participant.
1.2 I/We understand the Participant’s participation is completely voluntary.
1.3 I/We do hereby affirm and acknowledge that I/we have been fully informed of the inherent
hazards and risks associated with the Event. In consideration of the Participant being permitted
to participate in the Event, I/we, hereby agree(s) to the following:
1.3.1 Participation: Permission is granted for the Participant to participate in the Event with the
understanding that the participation is not required but is voluntary.
1.3.2 Risks: I/We understand and acknowledge that certain risks are inherent in participating in
all sports or sporting events, including the Event, and that those risks include but are not
limited to:
a) colliding, hitting, rushing, running into, or making physical contact with opposing
players, teammates, coaches, athletic staff, referees, or spectators;
b) falling, diving, sliding, running into, or making physical contact with the ball, ground,
wall, objects, or athletic equipments;
c) minor injuries such as scratches, bruises, and sprains;
d) major injuries such as eye injury or loss of sight, neck or spinal cord injuries, joint or
back injuries, concussions, serious brain damage, fractures, ligament and/or cartilage
damage, and injury to virtually all internal organs;
e) catastrophic injuries including paralysis and death;
f) the dangers inherent in any type of transportation, including driving in private cars,
buses, or any type of private or public transportation to and from the Event; and
g) the dangers being in a place open to the public.
1.4 I/We also understand and acknowledge that there are certain additional risks inherent in the
Event, including but not limited to:
1.4.1 exposure to harsh or extreme weather, climate, or environment;
1.4.2 exposure to bug bites; and
1.4.3 dangers in colliding with another Participant.
1.5 I/we assume liability and responsibility for any such risks associated with the participation in the
Event, whether described in this Release, known or unknown, inherent or otherwise.
1.6 I/We understand and acknowledge that the Participant is expected to abide by all rules and/or
instructions of the Event Organisers during the course of the Event and conduct him or herself in
an exemplary social manner and display good sportsmanship at all time. I/We are also aware that
participants are recommended to bring with them appropriate sports shoes to participate in
physical activities on both an outdoor grass pitch (shoes with cleats would be appropriate) and an
indoor sports hall (shoes with cleats would not be appropriate).
1.7 I/We agree to direct the Participant to comply with all directions and instructions of the Event
Organisers and coaches or other supervisory personnel in charge of the Event. I/We understand
that any of the Event Organisers has the authority to remove the Participant from the Event and
deny the Participant’s participation in the Event. The Event Organisers expect parents and legal
guardians to model polite behaviour and show support for effort and good play. The Event
Organisers, coaching staff, referees, teammates, and opposing players and coaches are to be
treated with respect at all times.
1.8 I/We acknowledge that, as a condition of the Participant’s participation in the Event, I/we hold
harmless and waive any and all claims against the Event Organisers, including but not limited to,
claims arising out of any ordinary negligence of any of the Event Organisers, or any loss or damage
to personal property occurring during or by reason of the Participant participating in the Event.
1.9 I/We understand that each of the Event Organisers is not an agent of, and has no responsibility
for, any third party including without limitation any sponsor or program that may provide any
service, equipment, training or activities associated with the Event.
1.10 As a condition of the Participant’s participation in the Event, I/we indemnify the Event Organisers
for all claims that may arise from other individuals, entities, or other third parties, as a result of
the Participant’s participation in the Event and/or conduct during the Event, including but not
limited to any injury, accident, illness, or death, or any loss or damage to personal property.
1.11 I/We consent to any of the Event Organisers administering such emergency medical care to the
Participant during the Event, as such person deems appropriate in the circumstances, and hereby
authorize medical treatment in case of emergency and to provide all relevant information
requested.
1.12 I/We understand and acknowledge that the Event Organisers do not carry or maintain health
medical, or disability insurance coverage for the Participant and therefore I/we agree to assume
the responsibility for such insurance coverage on the Participant for anything that occurs during
the Event.
1.13 Prior to the Participant’s participation in the Event, I/we agree to provide the Event Organisers
with complete and accurate medical or health-related information concerning the Participant,
particularly in relation to any medical attention the Participant may need during the course of
the Event. This includes, but is not limited to, allergies, asthma, and medication, of the
Participant, and names and phone numbers the Participant’s emergency contact.
1.14 If any provision of this Release is held invalid or unenforceable, the remainder of this Release
shall nevertheless remain in full force and effect. If any provision is held invalid or unenforceable
with respect to particular circumstances, it shall nevertheless remain in full force and effect in all
other circumstances.
1.15 Event Media & Personal Data
1.15.1 Visual and audio-visual recordings of the Participant and/or accompanying
parent(s)/legal guardians(s), whether in analogue or digital format, including but not
limited to photographs, videos, and audio and film recordings, and in any form of
technology (“Event Media”) may be captured during the course of the Event. In
consideration of the Participant’s participation in the Event, I/we hereby: (i) consent to
the Event Organisers capturing Event Media, (ii) grant the Event Organisers and their
respective designees, partners or vendors an irrevocable, world-wide, non-exclusive,
sub-licensable, royalty-free license to make, take, use, reproduce, edit, copy, add to,
adapt, translate, dub and/or refer to the Event Media in any manner and in, by or
through any and all media, for any marketing, advertising or promotions, whether
telecast, published, or used in other medium (including but not limited to posting on
the world wide web), and to maintain such Event Media and reproductions of the same
in a database for future use, all without additional remuneration, all to the fullest extent
permitted by law.
1.15.2 All Event Media and re-production(s) of same in any form, are properties of the Event
Organisers.
1.15.3 I/We release the Event Organisers and their respective partners, vendors, and their
officers, employees, representatives, agents and volunteers from any and all claims,
demands, costs and liabilities that may arise from the use of the Event Media and re-
productions of the same.
1.15.4 I agree that WFL may collect, use and disclose my personal data, which I have
provided in this form, for purposes of (i) monitoring access; (ii) health and safety
purposes; and (iii) emergency contact purposes, in accordance with the Personal Data
Protection Act 2012
1.15.5 I agree that WFL may share my personal data, which I have provided in this form,
with the Football Association Premier League for the purposes of: (i) event
administration; (ii) health and safety; and (iii) emergency purposes.

I/WE HAVE READ THIS RELEASE AND I/WE FULLY UNDERSTAND THE TERMS. I/WE KNOW OF THE
INHERENT RISKS AND DANGER INVOLVED IN PARTICIPATING IN THE EVENT AND AGREE THAT THE
PARTICIPANT VOLUNTARILY EXPOSES HIM OR HERSELF TO THOSE RISKS. THIS IS TO CERTIFY THAT I/WE,
AS PARENT(S) OR LEGAL GUARDIAN(S) OF THE CHILD, DO CONSENT AND AGREE THAT THE PARTICIPANT
CAN PARTICIPATE IN THE EVENT PURSUANT TO THE TERMS OF THIS RELEASE.
By confirming the participant’s participation in the event, I/We have read, understood and
unconditionally agreed to the above.
